Though, my all time favourite crit event story from safety classes, and a great example of how quickly something can go from a safe configuration, to unsafe, to safe and back to unsafe was the one that happened in 1958. Though, what is missing from the pdf is the part where when they turned of the stirrer, it caused the fluid in the vat to return to an unsafe configuration and re-crit.
